      The date I saw on the form was submission by 08/03/21. Is there a different form somewhere?

        Hey Cathy! I don’t see the 08/03/21 date anywhere in the rebate form. The last date when reimbursement requests are accepted is 4/3/22. Hope that helps! 🙂

      I downloaded thie mail in rebate form and it doesn’t say anything about having to be from Amazon. Can I purchase the Tylenol product at another store since the rebate form doesn’t say anything about having to be purchased from a certain store

      The terms say that you must use cash or cash equivalent – does that mean credit card is okay?

        I would imagine so! It says no points or other non-monetary purchase methods and credit card is a monetary payment method so I’m sure it’s fine!
